Gastrointestinal hormones called incretins such as Glucagon-like peptide-1 also known as GLP-1, lowers the blood sugar levels by stimulating the production of insulin when it is needed (for example after eating) and reducing the production of glucagon by the liver when it is not needed (for example during digestion). They also slow down digestion, decrease appetite and reduces weight. Based on these beneficial effects of Glucagon-like peptide-1, certain Glucagon-like peptide-1 receptor agonists, also known as GLP-1 receptor agonists or incretin mimetics, were developed for treatment of hyperglycemia in diabetic patients. Medications in these class includes, exenatide (trade names Byetta and Bydureon), liraglutide (trade names Victoza and Saxenda), lixisenatide (trade name Lyxumia), albiglutide (trade name Tanzeum), dulaglutide (trade name Trulicity) and Semaglutide. Semaglutide (trade name, Ozempic) is a medication developed by Danish company Novo Nordisk. Originally developed for the treatment of type 2 diabetes in 2012. Injection version of the drug has been approved by the US Food and Drug Administration (FDA) in 2017, and by the European Commission, Health Canada and the Japanese Ministry of Health, Labour and Welfare in 2018. On 20 Sep 2019, The U.S. Food and Drug Administration, approved Novo Nordisk's Rybelsus (containing semaglutide) oral tablets, to improve control of blood sugar in adult patients with type 2 diabetes, along with diet and exercise. The efficacy and safety of Rybelsus in reducing blood sugar in patients with type 2 diabetes were studied in several clinical trials, two of which were placebo-controlled and several of which were compared to other GLP-1 injection treatments. Rybelsus was studied as a stand-alone therapy and in combination with other diabetes treatments, including metformin, sulfonylureas (insulin secretagogues), sodium-glucose co-transporter-2 (SGLT-2) inhibitors, insulins and thiazolidinediones, all in patients with type 2 diabetes. In the placebo-controlled studies, Rybelsus as a stand-alone therapy resulted in a significant reduction in blood sugar (hemoglobin A1c) compared with placebo, as determined through HbA1c tests, which measure average levels of blood sugar over time. Recommended dose of Rybelsus: Rybelsus can be started with 3 mg once daily for 30 days. After 30 days on the 3 mg dose, it can be increased to 7 mg once daily. Dose may be increased to 14 mg once daily if additional glycemic control is needed after at least 30 days on the 7 mg dose. It is not recommended as first-line therapy for patients inadequately controlled on diet and exercise and Not indicated for use in patients with type 1 diabetes mellitus or treatment of diabetic ketoacidosis. Adverse reactions and Contraindications: The most common adverse reactions, reported in patients treated with Semaglutide are gastrointestinal side effects including nausea, abdominal pain, diarrhea, decreased appetite, vomiting and constipation.
